Case materials can be most easily submitted with our laboratory's standard form. If your laboratory is not already supplied with these, you can click here to display the form in Adobe® PDF format and print a copy. Information for Dr. Banks from the submitting pathologist can be included on the bottom portion of the form, or in a letter of enclosure. Client billing will be utilized unless consultations are requested by patients or their managing physicians, in which case patient billing information is required.

If frozen tissue is included (on dry ice) or if blood specimens are being sent for flow cytometry analysis (on wet ice) materials should not be sent on Friday or Saturday, in order to ensure that these are received during office hours at Celligent Diagnostics.

As soon as cases are completed the consultative letter is faxed to the submitting pathologist. Any special stains performed in the consultative lab are returned to the sender with all original slides and paraffin blocks via a 2-day courier service with tracking.
